Unknown base band, iccd, imei and meid. Why and what to do?

I own a xoom that at one time was functioning on the 4g lte network and now it is not. I would like to know what happened but, that is not all that important. what I would really like to know is how to get it back and running on the network. The sim card is new and no I have not been playing around in the ROM. so if I cannot fix it by any easy means, is there a place in which to obtain the original Motorola firmware to reload on the device.

I have done a hard reset a soft reset and I have done a complete wipe and have done them again. At the moment a device that was able to roam nationwide is now limited to wi-fi only. This is sickening in my case. So please I am asking for help thank you.

    Information about tablet.

    running 4.1.2 kit kat
    UN-rooted
    build number jz054m
